Employee Vaccination Verification

All employees are required to verify their vaccination status or attest an eligible exemption to enter campus.

To attest your vaccination status or exemption:

In this tutorial, we will walk through how to complete your vaccine attestation.

This tutorial can only be done from a desktop or laptop computer.

It is important that you follow all steps carefully. If something isn't done correctly in this process, you may have a hold placed on your account.

Step 1

On the main dashboard, click the tile that says "Immunization Attestation"

On this page, in the "*Immunization" field; enter the option that pertains to you. If you have taken either the Moderna or Pfizer vaccine, two doses will be required.
